- Formerly Community Habilitation Support Services (CHSS)
- Serves individuals’ with intellectual and developmental disabilities who choose to remain the community
- Individual should be diagnosed before the age 22 and the diagnosis should be indefinite with an IQ <70
- Individual should have 3 or more functional impairments:
- Self-Care
- Learning
- Mobility
- Self-Direction
- Capacity for independent living
- Requiring 24 hour care
